Our Work

The Community Health & Dignity Initiative — our framework for lasting change.

The Framework

Community Health & Dignity Initiative (CHDI)

CHDI is our integrated delivery model. Every outreach combines education, screening and commodity distribution across four pillars — so a single visit to a community addresses immediate needs while planting the seeds of long-term behavioural change.

Each outreach is planned with community leaders, delivered by trained volunteers, and followed up through local Community Health Ambassadors who keep education and support alive between visits.

Where We Work

Phase 1: Nigeria. Next: the world.

Our Phase 1 strategy focuses on densely populated, underserved communities in Lagos State — where the need is urgent and a single outreach reaches hundreds of families.

  • Now — Lagos State LGAs: recurring outreaches, ambassador network, community renovations.
  • Next — expansion to additional Nigerian states with trained regional teams.
  • Beyond — a global expansion roadmap bringing CHDI to underserved communities worldwide.
